A volunteer application is good for a calendar year. If you had one done in the spring, you are good for the fall. 
If you need to fill out a new one please send your first and last name with a valid email address to [email protected]. We will then send you a link to fill one out electronically. This is the only way we do them now. We no longer use the paper forms.
